Regarding the use of the term "punctuation form" prior to Berger's 1990s 
works:

Although the application may be different here than when addressing 
Mozart's works, Patrick Gowers used the term in “Satie's Rose Croix 
Music (1891-1895),” Proceedings of the Royal Musical Association 92 
(1965-66): 1-25 (see p. 18ff.).
